I agree with you in that the history and appropriate data need to be in place for change. I think that is the assumption made by the article where, "When you see the need for change in your team or organisation...". I believe that vision would come from an historical perspective from within.
I agree that change is continuous, especially in today's business climate where the pace of change has accelerated rapidly. Unless it is an incremental change, such as introducing a revision to an existing process or system where the changes are contained, most change we introduce into an organization rarely has an opportunity to become instutionalized before the next wave hits. What I like about Bridge's model is that it provides a easily explainable process for guiding people through a change - a simplified change curve.
